Player Waiver & Release Agreement
This Player Waiver & Release (“Agreement”) is entered into by the parent or legal guardian of the participant (“Player”) in consideration of the Player’s participation in programs, training sessions, camps, clinics, games, tournaments, and other activities organized by Koca Soccer Academy.
By completing the registration and accepting this Agreement, the parent or legal guardian acknowledges and agrees to the following:
1. Assumption of Risk
I understand that participation in soccer and related athletic activities involves inherent risks, including but not limited to falls, collisions, contact with other participants, equipment-related injuries, weather-related conditions, and other risks that may result in serious injury, permanent disability, or death.
I voluntarily assume all risks associated with the Player’s participation in Koca Soccer Academy activities.
2. Release of Liability
To the fullest extent permitted by applicable law, I release, waive, discharge, and hold harmless Koca Soccer Academy, Coach Koca, its coaches, assistants, volunteers, employees, contractors, affiliates, facility owners, sponsors, and representatives from any and all claims, liabilities, demands, actions, damages, losses, costs, or expenses arising out of or related to the Player’s participation in academy activities, except where caused by gross negligence or willful misconduct.
3. Medical Authorization
I certify that the Player is physically able to participate in soccer activities.
I authorize Koca Soccer Academy to obtain emergency medical treatment for the Player if I cannot be reached in a timely manner.
I understand that I am responsible for any medical expenses incurred as a result of such treatment.
4. Medical Information
I agree to provide accurate information regarding any medical conditions, allergies, medications, physical limitations, or other health concerns that may affect the Player’s safe participation.
I understand that failure to disclose relevant medical information may increase the risk of injury.
5. Player Conduct
I understand that players are expected to demonstrate good sportsmanship, respect, teamwork, and appropriate behavior at all academy activities.
Koca Soccer Academy reserves the right to remove or dismiss any participant whose behavior is unsafe, disruptive, abusive, or inconsistent with academy expectations.
6. Parent/Guardian Responsibilities
I agree to:
- Provide accurate registration information.
- Ensure the Player arrives prepared for training.
- Notify Koca Soccer Academy of any medical changes.
- Pick up the Player promptly after scheduled activities.
- Maintain respectful conduct toward coaches, officials, players, and other families.
7. Weather, Cancellations & Schedule Changes
I understand that activities may be delayed, relocated, postponed, or canceled due to weather, field conditions, safety concerns, facility availability, or other circumstances beyond the control of Koca Soccer Academy.
Reasonable efforts will be made to notify participants of schedule changes.
8. Insurance
I understand that Koca Soccer Academy does not provide medical or accident insurance for participants.
I acknowledge that I am responsible for maintaining appropriate health and accident insurance coverage for the Player.
9. Emergency Contact
I certify that the emergency contact information provided during registration is current and accurate.
I agree to notify Koca Soccer Academy promptly if this information changes.
10. Severability
If any provision of this Agreement is determined to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
11. Governing Law
This Agreement shall be governed by and interpreted in accordance with the laws of the State of Maryland.
12. Electronic Acceptance
By submitting the online registration form, I acknowledge that:
- I have carefully read this Player Waiver & Release.
- I fully understand its contents.
- I am the parent or legal guardian of the Player, or I have the legal authority to execute this Agreement on the Player’s behalf.
- I voluntarily agree to all terms contained herein.
- My electronic acceptance has the same legal effect as my handwritten signature.
